Community Psychiatric Nurse (CPN) Dementia Services

\n Reporting To: Team Lead / Service Manager, Dementia Services

About the Role
\n An experienced Community Psychiatric Nurse is sought to join a Dementia Services team in Shrewsbury, delivering high-quality, person-centred care to people living with dementia and their families/carers within the community. This is a great opportunity to work as part of a supportive multidisciplinary team, providing assessment, care planning and ongoing support to a caseload of patients.

Key Responsibilities

\n
• Carry out comprehensive assessments of patients with dementia and associated mental health needs
\n
• Develop, implement and review individualised care plans in the community
\n
• Provide support, education and advice to patients, families and carers
\n
• Work collaboratively within a multidisciplinary team (MDT), including psychiatrists, OTs, social workers and GPs
\n
• Monitor and manage medication, including side effects and concordance
\n
• Respond to crisis situations and risk assess appropriately
\n
• Maintain accurate clinical records in line with NMC and trust requirements
\n
• Support junior staff and contribute to training/development within the team
\n

Essential Requirements

\n
• Registered Nurse (RMN/RNLD or equivalent) with current NMC registration
\n
• Experience working within community mental health/dementia services
\n
• Sound knowledge of dementia care, risk assessment and safeguarding
\n
• Strong communication and organisational skills
\n
• Full UK driving licence (community role)
\n
• Right to work in the UK
\n
