Primary Care Provider (Rhode Island)

• Nation-wide primary care organization geared towards the geriatric population seeking Primary Care Advanced Practice Providers for their Rhode Island locations.

• Monday-Friday 8am-5pm schedule (no nights or weekends)
\n
• 12-16 patients per day once fully ramped up (could start lower if new site)
\n
• Patient population primarily medicare (over age 65) but see medicaid too so some sites have up to 25% under age 65
\n
• Collaborative care team - 1 MD, APP, Scribe, MA, Shared RN/Social Work support
\n
• Specialty support from partnership with outside telemedicine company (ex - can send images to a Derm, Cardiologist, GI, etc. and get response in a few hours, company called Rubicon)
\n
• EPIC EMR (scribes handle all of this, have advanced algorithm that helps predict how often patient needs to come back, have kept hospitalizations down 54% with this model)
\n
• Call schedule rotated amongst market, on average 1 week every 3 months (will be less as hire more/open more clinics) - have call center to triage so calls are limited and 100% remote

This is a full time permanent position with a 4-6 week orientation/training period, stock options, extensive PTO, large CME stipend, malpractice coverage and 401k with match available from day one **** Requirements: Board Certification Required RI State License Required Training as a Family NP, Adult NP to be considered Previous primary care or skilled nursing facility/nursing home experience preferred, but not required Sell for this client: Mobile vans bring patients to and from clinics, access to care for geriatric patients. Nationally ranked & publicly traded, started in Illinois and model really successful, going to continue to expand in NYC (Long Island, Staten Island, etc.). Very standardized training. Biggest selling point is patient volume, even when a site is at it's peak business, individual provider won't see over 16 per day. HUGE focus on quality of care vs quantity. Call this a "value-based care model ". Goal of company is to help keep Medicare (over age 65) patients out of the hospital.
