POSITION OVERVIEW

Our Nurse Practitioners (NPs) and Physician Assistants (PAs) are collaborative contributors to our organization’s success, assisting our partners in improving the quality of life for their residents. Key responsibilities include:

• Evaluating patients’ medical and mental health requirements and developing a care plan that proactively addresses their conditions and overcomes barriers to care.
• Ensuring adherence to regulations set by local, state, and federal agencies concerning the clinical services provided.
• Prescribing medications and arranging lab tests, diagnostic procedures, and consultations.
• Tracking patients’ adherence to treatment plans and their responses, making necessary adjustments as needed.

EXPERIENCE, CREDENTIALS, AND EDUCATION

• Active APRN or PA license is mandatory.
• Degree from an accredited APRN or PA educational program.
• A minimum of 3 years of experience in clinical nursing or rehabilitation, particularly with geriatric populations, and/or experience in Fee-for-Service settings.
• Familiarity with Electronic Health Records.
• Proficient in Microsoft Office applications, including Word, Outlook, and Excel.

SUPERVISORY DUTIES

• May involve providing training and guidance to facility staff.
