Physical Therapist

We are looking for a dedicated and empathetic Physical Therapist to become a part of our Healthcare & Medical Services team. This position is based on-site, where you will deliver evidence-based rehabilitation services to our senior residents across independent living, assisted living, and memory care settings.

• Conduct thorough assessments of residents' physical needs.
• Design and implement personalized treatment plans tailored to individual requirements.
• Develop and oversee fall-prevention and mobility enhancement programs.
• Collaborate closely with nursing and care staff to ensure comprehensive resident care.
• Provide education to residents and their families regarding treatment and wellness.
• Maintain accurate documentation of care within electronic health record (EHR) systems.
