About Our Facilities

Our organization operates as Skilled Nursing Facilities (SNFs) that offer:

• Long-term care: Providing ongoing skilled support and compassionate daily assistance to residents.
• Short-term, sub-acute rehabilitation: Assisting patients in recovery from surgery, illness, or injury.
• Collaborative care: Utilizing an interdisciplinary team approach with nursing, therapy, and medical staff working in unison.

The patient demographic may vary slightly by facility. As a therapy professional, you will play a crucial role in helping patients achieve functional improvements and enhance their quality of life in a supportive, patient-centered environment.

Position Overview

The Physical Therapist (PT) delivers direct rehabilitation care in physical therapy to patients referred by physicians, adhering to all relevant laws, regulations, and organizational standards.

Qualifications

• Must be a graduate of an accredited physical therapy program (BSPT, MSPT, or DPT); those graduating in 2003 or later must possess an MSPT or DPT.
• Preferred: At least one year of experience in a post-acute care or similar setting.
• Engagement in community health initiatives/projects as appropriate.
• Proficient in Microsoft Word, Excel, and email communication.
• Ability to liaise effectively with patients, families, and support departments.
• Knowledgeable about practices, procedures, and regulations governing operations in post-acute care facilities.

Responsibilities

• Implement and interpret departmental programs, goals, objectives, policies, and procedures.
• Demonstrate proficiency in all competency areas, including patient rights, safety, and sanitation.
• Maintain confidentiality of all proprietary and confidential information.
• Understand and adhere to organizational policies, including those related to harassment and compliance.
• Exhibit integrity and professionalism by following the organization's Code of Conduct and completing mandatory compliance training.
• Follow prescribed physical therapy treatment plans for patients.
• Oversee and evaluate the care provided by Physical Therapy Assistants (PTAs), Rehabilitation Aides, and PT and PTA students.
• Utilize therapy software accurately and appropriately.
• Provide excellent customer service and maintain a positive attitude towards patients.
• Read, write, speak, and comprehend the English language effectively.
