Our organization operates Skilled Nursing Facilities (SNFs) that offer:

  • Long-term care: Providing ongoing skilled support and compassionate daily assistance to residents.
  • Short-term, sub-acute rehabilitation: Assisting patients in their recovery from surgery, illness, or injury.
  • Collaborative care: Utilizing an interdisciplinary team approach where nursing, therapy, and medical staff work in unison.

The patient demographic may vary slightly by facility. As a therapy professional, your role will be to support patients in achieving functional improvements and enhancing their quality of life in a nurturing, patient-focused environment.

Position Overview

The Physical Therapist (PT) delivers direct rehabilitation services in physical therapy to patients referred by physicians, adhering to all relevant laws, regulations, and organizational standards.

Qualifications
  • Graduation from an accredited physical therapy program (BSPT, MSPT, or DPT; those graduating in 2003 or later must possess an MSPT or DPT).
  • Preferred: At least one year of experience in a post-acute care or similar setting.
  • Active participation in community health initiatives/projects as appropriate.
  • Proficiency in Microsoft Word, Excel, and email communication.
  • Ability to liaise effectively with patients, families, and support departments.
  • Familiarity with the practices, procedures, laws, regulations, and guidelines relevant to post-acute care facilities.
Key Responsibilities
  • Implement and interpret departmental programs, goals, objectives, policies, and procedures.
  • Demonstrate proficiency in all competency areas, including patient rights, safety, and sanitation.
  • Maintain confidentiality regarding all proprietary and confidential information.
  • Understand and adhere to organizational policies, including those related to harassment and compliance.
  • Exhibit integrity and professionalism by following the organization's Code of Conduct and completing mandatory compliance training.
  • Follow established physical therapy treatment plans for patients.
  • Supervise and assess the care provided by Physical Therapy Assistants (PTAs), Rehabilitation Aides, and PT and PTA students.
  • Utilize therapy software accurately and effectively.
  • Provide excellent customer service and maintain a positive attitude towards patients.
  • Read, write, speak, and comprehend the English language proficiently.
