This position involves evaluating mobility and functional requirements, developing tailored treatment strategies, and implementing evidence-based therapeutic interventions aimed at enhancing strength, balance, and independence.

Key Responsibilities

• Conduct assessments to determine residents' mobility and functional needs.
• Create personalized treatment plans based on individual assessments.
• Provide evidence-based therapies to promote physical strength and balance.
• Work collaboratively with nursing and caregiving staff to ensure comprehensive care.
• Document all care activities in the Electronic Health Record (EHR) system.
• Educate residents and their families on safety practices and home exercise routines.

Qualifications and Requirements

• Valid Physical Therapy license.
• Experience in geriatric care is preferred.
• Strong communication and interpersonal skills.
• Ability to work effectively in a team-oriented environment.
