Director of Case Management Opportunity:\n

• Full-time permanent position
\n
• Director of Case Management manage the daily operations and HR functions of the Case Management department
\n
• Directs the interdisciplinary care, discharge planning, and patient outcomes while championing patient advocacy
\n
• Guides and trains staff, builds relationships with healthcare and insurance networks, and fosters a positive, celebratory work culture
\n

Director of Case Management Qualifications: \n

\n
• Must be a licensed clinician (ex: RN, SW, OT, PT, ST, Rehabilitation counseling, etc.)
\n
• Current CCM or ACM certification or eligibility to obtain within first year of employment
\n
• Must possess a bachelor's degree; graduate degree is preferred
\n
• Three years of hospital-based Case Management experience, including Utilization Review and Discharge Planning experience
\n
