Your Responsibilities

Deliver Exceptional Patient Care
  • Administer direct physical therapy services under the supervision of a licensed Physical Therapist.
  • Implement individualized care plans to assist patients in alleviating pain, enhancing mobility, and restoring function.
  • Guide patients through therapeutic exercises, gait training, balance activities, and functional mobility.
  • Support patients with transfers, strength training, and daily living activities.
  • Utilize therapeutic modalities such as heat, cold, ultrasound, and electrical stimulation as needed.
  • Educate patients and their caregivers on exercises, safety measures, and the use of assistive devices.

Monitor Progress & Communicate
  • Observe and document patient progress, treatment responses, and any changes in condition.
  • Maintain accurate and timely records in the electronic medical record (EMR) system.
  • Effectively communicate with the Physical Therapist and the interdisciplinary care team.
  • Engage in case conferences and care coordination activities.

Collaborate & Contribute
  • Work closely with nurses, therapists, and other healthcare professionals to ensure cohesive care.
  • Participate in team meetings, training sessions, and ongoing educational opportunities.
  • Adhere to clinical standards and contribute to quality improvement initiatives.

Your Qualifications
  • Graduation from an accredited Physical Therapy Assistant (PTA) program.
  • Current PTA license or certification as mandated by state regulations.
  • Preferred experience of 1+ years (home health experience is a plus).
  • Current CPR certification.
  • A valid driver’s license, reliable transportation, and auto insurance.
