Job Description
Doctor of Pharmacy (PharmD) degree, active Pharmacist license, and minimum 1+ years Pharmacist experience required. Applicants who do not meet these qualifications will not be considered.
A home infusion clinical pharmacist plays a vital role in preparing, dispensing, and monitoring complex intravenous (IV) and subcutaneous medications for patients receiving care at home. This position acts as the central coordinator within a multidisciplinary team, ensuring accurate sterile compounding, precise medication adjustments, and comprehensive patient education to promote effective therapy and patient safety.

Type: Contract (8-12 weeks)

Schedule:
- Monday to Friday, 8:30 a.m. – 5:00 p.m.
- May require participation in on-call rotation
Responsibilities:
- Prepare specialty pharmaceuticals and IV solutions in sterile, ventilated environments per state and federal regulations
- Monitor lab values, calculate pharmacokinetic dosing (e.g., vancomycin), and oversee Total Parenteral Nutrition (TPN) adjustments
- Provide detailed counseling and education to patients and caregivers on medication usage, infusion pumps, sterile techniques, and catheter maintenance
- Collaborate closely with prescribing physicians, home health nurses, and internal teams to coordinate care and prevent hospital readmissions
- Ensure compliance with all relevant regulations including State Board of Pharmacy, HIPAA, FDA, DEA, Medicare, ACHC, URAC, OSHA
- Manage pharmacy inventory, including refrigerated products, and prepare medications for safe home delivery
Qualifications:
- 1–3 years of experience in infusion specialty pharmacy or hospital pharmacy with primary responsibility in the IV room
- Experience with IVIg is a plus
- Knowledge of specialty therapy payer requirements, reimbursement, and contract obligations
- Current pharmacist license in Arizona (and Texas if applicable), in good standing
- Familiarity with CareTend system preferred
