Job Duration: 13 weeks

Shift: Day 5x8-Hour (08:00 - 16:30)

Patient Age Groups : Adults, Geriatrics

\n

Daily Responsibilities:

\n

\n
• Develop and implement care plans for patients
\n
• Provide skilled nursing care and interventions
\n
• Administer IV therapy and medications
\n
• Assess patient needs and vital signs
\n
• Collaborate with healthcare team to ensure quality patient care
\n

Required Skills:

\n

\n
• Inpatient Acute Case Management (Case Manager Hospital experience)
\n
• MUST HAVE DISCHARGE PLANNING EXPERIENCE
\n
• BLS, CA RN License
\n
