Oncology Pharmacist | Dover, Delaware

A full-time Oncology Clinical Pharmacy Specialist position is available for a direct hire opportunity starting ASAP at a hospital facility located in Dover, DE. This role focuses on providing expert pharmaceutical care to cancer patients throughout their treatment journey, ensuring optimal therapy management in collaboration with the healthcare team. The day shift schedule consists of 80 hours per pay period.

Responsibilities:

• Develop and maintain collaborative practice programs with oncologists, addressing areas of need including supportive care for chemotherapy patients both inpatient and outpatient.
• Deliver educational programs to pharmacy staff, nursing, oncologists, and other healthcare professionals.
• Act as a subject matter expert in oncology by responding to drug information inquiries and ensuring EHR oncology care plans align with current national guidelines.
• Participate in rounds with Oncology Fellows, Nurse Practitioners, and Physicians to assess and optimize medication therapies based on evidence-based guidelines and institutional policies.
• Provide patient-specific medication education, including the teach-back method for new oral oncolytics and intravenous chemotherapy when necessary.
• Continuously review and update oncology care plans within the Electronic Health Record.
• Identify and assess medication-related problems through clinical data analysis.
• Precept pharmacy residents and students.
• Engage actively in Oncology Service Line meetings to enhance cancer care safety, efficacy, and quality.
• Promote rational, cost-effective drug therapies to achieve cost avoidance and savings within the oncology service line.
• Perform other duties consistent with the position’s scope.

Qualifications:

• Doctorate in Pharmacy required.
• At least three years of hospital experience including oncology practice; or completion of PGY-2 ASHP-accredited oncology residency or equivalent experience.
• Licensed Pharmacist with Board Certification in Oncology Pharmacy.
• Preferred candidates have completed a PGY-2 residency or possess five years of related experience with BPS oncology board certification.
