The Medical Social Worker, under the supervision of the Clinical Manager, plays a vital role in the interdisciplinary care provided to patients receiving home health services. Your primary responsibility will be to assess and create a tailored care plan that addresses the emotional and social needs of each patient. Additionally, you will oversee and guide the Social Worker Assistant involved in the patient's care plan. This position requires adherence to our organization's policies, healthcare standards, and relevant laws and regulations.

Key Responsibilities

• Evaluate the social and emotional well-being of patients in relation to their medical conditions, care requirements, and treatment responses.
• Examine the interplay between patients' medical and nursing needs, financial resources, and community support available in their home environment.
• Take necessary actions to access community resources that can assist in overcoming barriers to the patient's recovery.
• Engage in communication with patients, their families, and colleagues within the organization.
• Continuously assess the needs of patients and families, focusing on their understanding of the treatment process and coping with emotional and social challenges.
• Contribute as a member of the interdisciplinary care team, facilitating care coordination and serving as a resource for identifying and addressing patient needs.
• Monitor the performance of the Social Work Assistant (BSW) to ensure compliance with company policies, governmental regulations, and professional standards in delivering medical social services.
• Maintain and submit required documentation, including case conferences, patient and physician community contacts, visit reports, and progress notes, while collaborating with other healthcare professionals to ensure optimal patient care.

Qualifications and Requirements

• A minimum of one year of experience as a social worker in a healthcare environment, including home health or hospice settings.
• Licensure as a Social Worker in the state of practice, if mandated by state regulations.
• A valid driver's license, auto insurance, and dependable transportation.
• Current CPR certification.
• Experience in discharge planning and accessing community resources (e.g., housing, shelter, legal services, financial assistance programs).
• Knowledge of medications and their proper administration.
• Proven ability to organize tasks, create action plans, prioritize effectively, and perform under pressure.
• Willingness to work flexible hours and travel locally as needed.
• Maintain current licensure and fulfill mandatory continuing education requirements.
• Fluency in reading, writing, and speaking English.

Additional Information

TB Screening: This position involves direct patient interaction and is part of our Tuberculosis (TB) screening program. If selected, you will be required to undergo TB screening.

Driving Requirements: This role is included in our driver safety program, necessitating a valid state driver’s license and personal vehicle liability insurance. You must maintain insurance that meets or exceeds your state’s minimum requirements, or at least $25,000 for bodily injury per person, $25,000 for bodily injury per event, and $10,000 for property damage, whichever is higher.

The salary range provided below represents a good faith estimate of the starting base pay for full-time (40 hours per week) employment at the time of posting. Actual pay may vary based on geographic location and individual qualifications, including skills, experience, education, and certifications.

Salary Range: $53,700 - $72,600 per year
