About Our Facilities

Our organization operates as Skilled Nursing Facilities (SNFs), providing:

• Long-Term Care: Ongoing skilled support and compassionate daily assistance for residents.
• Short-Term Rehabilitation: Sub-acute care for patients recovering from surgery, illness, or injury.
• Collaborative Care: An interdisciplinary team approach involving nursing, therapy, and medical staff working in unison.

As a therapy professional, you will play a vital role in helping patients achieve functional improvements and enhance their quality of life in a supportive, patient-centered setting.

Key Responsibilities

• Provide direct rehabilitation care in physical therapy to patients based on physician referrals, under the supervision of a licensed Physical Therapist (PT).
• Participate in community health initiatives and projects as needed.
• Implement and interpret departmental programs, goals, objectives, policies, and procedures.
• Maintain proficiency in all competency areas, including patient rights, safety, and sanitation.
• Ensure confidentiality of all proprietary and confidential information.
• Adhere to company policies, including those related to harassment and compliance.
• Follow physical therapy treatment plans as directed by the supervising PT.
• Utilize therapy software accurately and effectively.
• Exhibit excellent customer service and maintain a positive attitude towards patients.

Qualifications

• Graduate of an accredited physical therapy program (ASPT).
• New graduates are welcome to apply.
• Proficiency in Microsoft Word, Excel, and email communication.
• Knowledge of practices, procedures, and regulations governing post-acute care facilities.
• Ability to read, write, speak, and understand the English language.
