Key Responsibilities:
- Collaborate with oncologists to develop and manage supportive care for patients undergoing chemotherapy in both inpatient and outpatient settings.
- Deliver educational programs to pharmacy and nursing staff, oncologists, and other healthcare professionals.
- Serve as the oncology subject matter expert, addressing drug information queries and reviewing electronic health record (EHR) care plans for consistency with national standards.
- Participate in multidisciplinary rounds to optimize cancer treatment plans, ensuring medication accuracy, dosage appropriateness, and therapeutic efficacy.
- Provide patient-specific medication education, employing the teach-back method for oral and intravenous chemotherapy.
- Review and update oncology care plans in the EHR system to maintain clinical relevance and adherence to guidelines.
- Identify and resolve medication-related problems through clinical assessment and data analysis.
- Precept pharmacy residents and students, fostering professional development.
- Engage in Oncology Service Line meetings to enhance safety, efficacy, and quality of care.
- Promote cost-effective drug therapy with a focus on cost avoidance and savings within the oncology service line.
- Perform all other duties within the scope of the position as assigned.
Qualifications:
- Doctorate in Pharmacy (PharmD) required.
- Licensed Pharmacist with Board Certification in Oncology.
- Minimum of three years hospital experience with clinical training and three years in oncology practice.
- Preferred: Completion of PGY-2 ASHP-accredited residency in oncology or equivalent experience; extensive oncology practice with board certification.
