Job Description
Doctor of Pharmacy (PharmD) degree, active Pharmacist license, and minimum 1+ years Pharmacist experience required. Applicants who do not meet these qualifications will not be considered.
A home infusion clinical pharmacist is responsible for preparing, dispensing, and monitoring complex intravenous (IV) and subcutaneous medications for patients, serving as a vital link within a multidisciplinary healthcare team. This role focuses on sterile compounding, clinical monitoring, patient education, and care coordination to ensure effective and safe therapy management.

Job Type: Contract (8-12 weeks)

Schedule:
- Monday – Friday, 8:30 a.m. – 5:00 p.m.
- May require participation in an on-call rotation
Responsibilities:
- Prepare specialty pharmaceuticals and IV solutions in a sterile, ventilated environment adhering to state and federal regulations.
- Assess lab values, perform pharmacokinetic dosing calculations (e.g., vancomycin), monitor Total Parenteral Nutrition (TPN), and adjust treatments accordingly.
- Provide comprehensive patient counseling and education on specialty medications, infusion pumps, sterile techniques, and catheter maintenance.
- Collaborate with prescribing physicians, home health nurses, and internal teams to ensure seamless therapy management and reduce hospital readmissions.
- Ensure compliance with all applicable regulations including State Board of Pharmacy, HIPAA, FDA, DEA, Medicare, ACHC, URAC, and OSHA.
- Manage pharmacy inventory, including refrigerated products, and ensure safe, timely medication delivery to patients’ homes.
Desired Experience and Qualifications:
- 1-3 years’ experience in infusion specialty pharmacy or hospital pharmacy with primary responsibilities in the IV room.
- IVIg experience is a plus.
- Strong understanding of specialty therapy payer requirements, reimbursement processes, and contract obligations.
- Current, active pharmacist license in Arizona and Texas in good standing.
- Familiarity with CareTend system preferred.
