Hematology/Oncology, Nurse Practitioner or Physician Assistant - Sickle Cell Disease

We are seeking a dedicated Nurse Practitioner or Physician Assistant to join our Adult Sickle Cell Disease Program at Indiana University Health Medical Group.

Position Details:

\n
• Full-time, Day Shift
\n
• Monday - Friday, 8:00am-5:00pm
\n
• 40% Inpatient / 60% Outpatient
\n

Job Responsibilities:
\n Clinical Care
\n • Evaluate, diagnose, and manage adult patients with sickle cell disease across inpatient and outpatient settings.
\n • Provide follow-up care after hospitalization, including transitional care visits and ongoing disease management.
\n • Manage acute and chronic complications of sickle cell disease, including:

\n
• Vaso-occlusive pain crises
\n
• Acute chest syndrome
\n
• Chronic transfusion management
\n
• Iron overload
\n
• Disease-modifying therapies
\n
• Pregnancy-related sickle cell care
\n

• Participate in inpatient rounding at IU Health hospitals as needed.
\n • Provide urgent access visits and infusion-based care within the Sickle Cell Center of Excellence.
\n • Collaborate with community providers regarding specialized sickle cell care management.
Program Growth & Outreach
\n • Serve as a clinical resource for expansion of sickle cell services into additional IU Health hospitals and outreach locations.
\n • Assist in developing and supporting outreach clinics throughout the Indianapolis region and surrounding communities.
\n • Participate in patient education initiatives and community engagement efforts.
\n • Support transitions of care from pediatric to adult sickle cell services.
\n • Help develop standardized workflows and best practices throughout the health system.
Interdisciplinary Collaboration
\n • Work closely with the program's two existing Sickle Cell APPs to provide seamless, coordinated patient care.
\n • Collaborate with hematologists, infusion nurses, social workers, pharmacists, case managers, and inpatient teams.
\n • Participate in multidisciplinary case reviews and quality initiatives.

Qualifications:
\n • Experience in Hematology, Oncology, Internal Medicine, Hospital Medicine, or Complex Chronic Disease Management.
\n • Experience caring for underserved or medically complex populations.
\n • Interest in program development, quality improvement, and healthcare innovation.

\n • Strong critical thinking and clinical decision-making skills.
\n • Flexibility to provide care across multiple sites and settings.
