Doctor of Pharmacy (PharmD) degree, active Pharmacist license, and minimum 1+ years Pharmacist experience required. Applicants who do not meet these qualifications will not be considered.

A contract role is available for a Staff Clinical/Infusion Pharmacist specializing in home infusion located in Richmond, TX. This position requires expertise in preparing, dispensing, and monitoring complex intravenous (IV) and subcutaneous medications while playing a vital role as the clinical hub for a multidisciplinary care team. The assignment begins ASAP and lasts approximately 8-12 weeks with a Monday through Friday schedule from 8:30 AM to 5:00 PM. Candidates may also be asked to participate in an on-call rotation.

Key Responsibilities:

  • Prepare sterile compounded specialty pharmaceuticals and IV solutions using proper aseptic techniques and compliance with federal and state regulations.
  • Monitor lab values, perform pharmacokinetic dosing calculations (e.g., vancomycin), and oversee Total Parenteral Nutrition (TPN) to tailor patient treatment plans.
  • Provide comprehensive patient and caregiver education on medication administration, infusion pump use, sterile techniques, and catheter care at home.
  • Collaborate with prescribers, home health nurses, and various clinical and operational teams to ensure cohesive care coordination and reduce hospital readmissions.
  • Maintain regulatory compliance with State Board of Pharmacy, HIPAA, FDA, DEA, Medicare, ACHC, URAC, OSHA, and other standards.
  • Manage pharmacy inventory including refrigerated medications and prepare shipments for timely patient delivery.

Qualifications:

  • 1-3 years of infusion specialty pharmacy experience or hospital pharmacy background with primary responsibilities in an IV room.
  • IVIg experience is a plus.
  • Familiarity with specialty therapy payer requirements, reimbursement, and contract terms.
  • Current and in good standing TX pharmacist license.
  • Experience working with CareTend system preferred.
