Per Diem Physician - Post Acute & Senior Care Services (Lititz)
Job Description
We are looking for dedicated Per Diem adult and senior care physicians to enhance our expanding post-acute care team in the Lancaster/Lititz area. This role is vital in delivering exceptional and compassionate care to our elderly population. Our organization provides services across various skilled nursing facilities throughout the county.
We encourage applications from Primary Care physicians, hospitalists, and those with geriatric training.
Position OverviewThe selected candidates will:
- Deliver direct patient care across a range of facilities, including skilled nursing, rehabilitation, personal care/assisted living, independent living, and clinics.
- Collaborate with facility leadership, hospital discharge teams, care management, and other specialties to ensure high-quality care aligned with shared objectives.
- Engage in diverse healthcare fields, spanning general primary care, internal medicine, geriatrics, and acute rehabilitation services, enhancing their expertise through collaboration with various specialists.
- Supervise and work alongside Advanced Practice Providers.
- Utilize EPIC EMR and access online diagnostics, including PACS imaging and lab results.
- Participate in clinical rounding Monday through Friday, with appealing daily schedules and flexible work models. Overnight and weekend on-call duties are limited to phone coverage, shared equitably among partners and APPs, ensuring favorable ratios.

Ideal candidates will possess:
- Board certification or eligibility in Internal Medicine, Family Medicine, or Geriatric Medicine.
