Pharmacist | Mesa, Arizona

Provide expert pharmaceutical care as a Staff Clinical/Infusion Pharmacist on an 8-12 week contract starting ASAP. This role is based in Mesa, AZ, and involves working Monday through Friday from 8:30 a.m. to 5:00 p.m., with the possibility of participating in an on-call rotation.

Key Responsibilities:

• Sterile Compounding: Accurately prepare specialty pharmaceuticals and intravenous (IV) solutions in compliance with state and federal sterile environment standards.
• Clinical Monitoring: Evaluate lab values, perform pharmacokinetic dosing calculations such as for vancomycin, and monitor Total Parenteral Nutrition (TPN). Provide comprehensive counseling and education to patients regarding their medication regimens.
• Patient Onboarding & Education: Instruct patients and caregivers on infusion pump operation, sterile technique, and catheter care for at-home therapy.
• Care Coordination: Work collaboratively with prescribing physicians, home health nurses, and various clinical teams to ensure effective therapy management and reduce hospital readmissions.
• Regulatory Compliance: Uphold adherence to all applicable regulations including those from the State Board of Pharmacy, HIPAA, FDA, DEA, Medicare, ACHC, URAC, and OSHA.
• Inventory & Shipping: Manage pharmacy inventory, including refrigerated products, and oversee packing and shipping of medications to patients’ homes.

Qualifications:

• 1-3 years of experience in infusion specialty pharmacy or hospital pharmacy with significant IV room responsibilities
• Familiarity with IVIg therapy is a plus
• Understanding of specialty therapy payer requirements, reimbursement, and contract processes
• Current and valid AZ pharmacist license in good standing
• Experience working with the CareTend system preferred
