Lively Medical Center is seeking a Family Nurse Practitioner (FNP) to join our established outpatient primary care team. This is a full-time, outpatient-only position offering a consistent schedule and strong clinical support.The ideal candidate is committed to delivering comprehensive primary care across the lifespan and values working within a collaborative, team-based model.

Position Highlights:\n

    Outpatient-only practice

    • \n
  • \n

    Schedule: 7:00 AM 5:00 PM, 4 days per week.

    • \n
  • \n

    Patient Volume: 1822 patients per day

    • \n
  • \n

    Call: Rotating call schedule shared among 10+ providers

    • \n
  • \n

    Supported by dedicated clinical staff (CMA/LPN) for rooming and patient flow

    • \n
  • \n

    Provide comprehensive primary care services including:

      Routine wellness visits

      • \n
    • \n

      Medicare Annual Wellness Visits

      • \n
    • \n

      Acute care visits

      • \n
    • \n

      Hospital follow-ups

      • \n
    • \n

      Preoperative examinations

      • \n
    • \n

      Chronic disease management

      • \n
    • \n
Qualifications:\n

  • \n

    Current certification as a Family Nurse Practitioner

    • \n
  • \n

    Active and unrestricted state licensure

    • \n
  • \n

    DEA certification (or eligible)

    • \n

    Commitment to high-quality, patient-centered care
