Clinical Pharmacist - Dover, Delaware

Doctor of Pharmacy (PharmD) degree, active Pharmacist license, and minimum 1+ years Pharmacist experience required. Applicants who do not meet these qualifications will not be considered.

A health system is seeking a full-time Clinical Pharmacist specializing in Oncology to join their Sussex Campus Hospital team. This role focuses on comprehensive cancer patient care, collaborating closely with oncologists, fellows, and multidisciplinary healthcare professionals to optimize treatment plans and supportive care in line with national guidelines.

Key Responsibilities:

• Develop and maintain collaborative practice programs with oncologists, focusing on inpatient and outpatient chemotherapy supportive care.
• Provide education and training to pharmacy, nursing staff, and oncologists.
• Serve as an oncology drug information expert, ensuring Electronic Health Record (EHR) care plans comply with current standards.
• Participate in clinical rounds with Oncology Fellows, Nurse Practitioners, and Physicians, making recommendations for medication therapy management.
• Deliver patient-centered medication education on oral oncolytics and intravenous chemotherapy using effective communication techniques.
• Review and update oncology care plans in the EHR.
• Detect and manage medication-related problems through clinical assessment.
• Precept pharmacy residents and students.
• Engage in Oncology Service Line meetings to enhance safety, efficacy, and quality of care.
• Promote cost-effective drug therapy practices targeting cost avoidance and savings.

Qualifications:

• Doctorate in Pharmacy (PharmD) required.
• Completion of Postgraduate Year 1 Residency plus at least three years of clinical oncology pharmacy practice, or completion of Postgraduate Year 2 Oncology Residency.
• Licensed Pharmacist with Board Certification in Oncology.
• Minimum of three years hospital experience with oncology clinical training required.
• Preferred: PGY-2 ASHP-accredited residency or PGY-1 with substantial oncology experience and BPS board certification.
• Preferred: Minimum five years oncology experience with BPS board certification.

Schedule: Full-time, 80 hours per pay period, Day shift.
