Our organization is currently in search of a Primary Care Physician specializing in Family Medicine to become a part of our team at Horizon Family Medicine, situated in Hermitage, Pennsylvania.

• Horizon Family Medicine is located within the Hermitage Outpatient Center.
• Join a team where one physician and two APPs manage care for 14-18 patients daily.
• Be part of a large Family Medicine call group comprising 18 providers.
• Available resources at the practice and outpatient center include X-ray services, diabetic retinal exams, orthopedic care, rehabilitation services, and pediatrics.
• Mammography, lab, and X-ray facilities are conveniently located within half a mile of the Outpatient Center.

• Medical Degree (MD/DO)
• Completion of an accredited Family Medicine residency program
• Board Certification in Family Medicine (or board eligible for recent residency graduates)
• Unrestricted medical license in Pennsylvania
• DEA certification
