About Our Facilities

Our organization operates as Skilled Nursing Facilities (SNFs) that offer:

  • Long-term care: Providing ongoing skilled support and compassionate daily assistance to residents.
  • Short-term rehabilitation: Assisting patients recovering from surgery, illness, or injury through sub-acute rehabilitation.
  • Collaborative care: An interdisciplinary team approach involving nursing, therapy, and medical staff working in unison.

As a therapy professional, you will play a vital role in helping patients achieve functional improvements and enhance their quality of life in a supportive, patient-centered environment.

Key Responsibilities
  • Deliver direct rehabilitation care in physical therapy to patients based on physician referrals, adhering to all relevant laws, regulations, and organizational standards.
  • Participate in community health initiatives and projects as needed.
  • Maintain proficiency in Microsoft Word, Excel, and email communications.
  • Serve as a liaison among patients, families, and support departments, demonstrating knowledge of practices, procedures, and regulations governing post-acute care facilities.
  • Implement and interpret departmental programs, goals, objectives, policies, and procedures.
  • Exhibit proficiency in all competency areas, including patient rights, safety, and sanitation.
  • Ensure confidentiality of all proprietary and sensitive information.
  • Adhere to company policies, including those related to harassment and compliance.
  • Demonstrate integrity and professionalism by following the organization's Code of Conduct and completing all required compliance training.
  • Follow established physical therapy treatment plans for patients.
  • Supervise and assess the care provided by Physical Therapist Assistants (PTAs), Rehabilitation Aides, and PT and PTA students.
  • Utilize therapy software accurately and effectively.
  • Provide excellent customer service and maintain a positive attitude towards patients.
  • Communicate effectively in English, both verbally and in writing.
Qualifications
  • Graduation from an accredited physical therapy program (BSPT, MSPT, or DPT; those graduating in 2003 or later must possess an MSPT or DPT).
  • Preferred: At least one year of experience in a post-acute care or related setting.
