About Our Facilities

Our organization operates Skilled Nursing Facilities (SNFs) that offer:

• Long-term care: Support for residents needing ongoing skilled assistance and compassionate daily care.
• Short-term rehabilitation: Services for patients recovering from surgery, illness, or injury.
• Collaborative care: An interdisciplinary team approach involving nursing, therapy, and medical staff working in unison.

The patient demographic may vary slightly by facility. As a therapy professional, you will play a crucial role in helping patients achieve functional improvements and enhance their quality of life in a supportive, patient-focused environment.

Position Responsibilities

The Physical Therapist (PT) will:

• Provide direct rehabilitation care in physical therapy to patients referred by physicians, adhering to all relevant laws, regulations, and organizational standards.
• Participate in community health initiatives and projects as appropriate.
• Communicate effectively with patients, families, and support departments.
• Be knowledgeable about practices, procedures, and the laws governing operations in post-acute care settings.
• Implement and interpret departmental programs, goals, objectives, policies, and procedures.
• Demonstrate proficiency in all competency areas, including patient rights, safety, and sanitation.
• Maintain confidentiality of all proprietary and sensitive information.
• Understand and adhere to company policies, including those related to harassment and compliance.
• Exhibit integrity and professionalism by following the organization’s Code of Conduct and completing required compliance training.
• Follow established physical therapy treatment plans for patients.
• Supervise and assess the care provided by Physical Therapist Assistants (PTAs), Rehabilitation Aides, and PT and PTA students.
• Utilize therapy software accurately and effectively.
• Provide excellent customer service and maintain a positive attitude towards patients.
• Read, write, speak, and understand the English language proficiently.

Qualifications

• Graduate of an accredited physical therapy program (BSPT, MSPT, or DPT); those graduating in 2003 or later must possess an MSPT or DPT.
