Position Overview

At our organization, team members transform lives and instill hope through genuine care. As an Occupational Therapist, you will plan, organize, and deliver specialized therapy services that assist patients in maintaining comfort, function, and dignity during their end-of-life journey. Guided by our core values—trustworthy, capable, compassionate, proactive, and called—you will play a vital role in supporting patients and families through skilled interventions and compassionate care.

Key Responsibilities

Patient Care

• Assess the patient’s functional abilities, including muscle strength, endurance, coordination, communication skills, self-care capabilities, and work potential.
• Evaluate the home environment to identify hazards or barriers to independence and determine necessary adaptive equipment.
• Contribute to the development of the patient’s comprehensive care plan and establish personalized treatment programs and objectives.
• Teach new skills or retrain patients in daily living activities affected by illness or injury, in accordance with organizational policies.
• Fabricate splints and instruct patients on the use of adaptive equipment to enhance safety and functionality.
• Provide training for prosthetic or orthotic devices as needed.
• Educate family members, caregivers, and staff about the treatment plan and how to support the patient’s functional needs.
• Engage in discharge planning and facilitate smooth transitions in care.

Communication & Coordination

• Maintain accurate clinical documentation, notes, and reports; promptly inform physicians of any changes in patient condition as required.
• Communicate the care plan to the referring physician and other members of the care team.
• Ensure ongoing communication with all personnel involved in the patient’s care to guarantee coordinated and efficient service delivery.
• Participate in in-services, interdisciplinary group (IDG) meetings, and other required gatherings to support comprehensive care planning.

Additional Duties

• Supervise Certified Occupational Therapy Assistants (COTAs) in accordance with organizational policies and state regulations.
• Engage in orientation programs for hospice personnel.

Qualifications

• Graduate of an accredited occupational therapy program that meets the standards set by AOTA and AMA; fulfills personnel qualifications per §418.114(b)(5).
• Registered through the National Registration Examination of the American Occupational Therapy Association.
• A minimum of two years of clinical experience as an Occupational Therapist.
• Strong verbal, written, and organizational abilities.
• Current CPR certification.
• Licensed driver with a reliable, insured vehicle that meets organizational standards.
