GENERAL RESPONSIBILITIES: The Patient Access Coordinator serves as an integral part of the Practice Operations Team by providing specialized support to WYH locations. The Patient Access Coordinator is responsible for identifying and facilitating pre and post visit patient needs related to gaps in care and/or standards of care for WYH primary care practices and will act as a Subject Matter Expert in all administrative and support functions of a Practice Operations Team.
