A leading primary care group in Houston is hiring a bilingual Community Health Worker (CHW) to join their team. This is a patient-focused role centered around patient navigation, care coordination, and connecting patients with healthcare and community resources.

Job Details:

Pay: $52,000-$58,240/year
Schedule: Monday-Friday, 8:00 AM-5:00 PM
Location: Houston, TX | 100% Onsite
Employment Type: Direct Hire | Full-Time
Language: Bilingual English/Spanish Required

Job Duties:

• Manage a group of patients and help coordinate their care.
• Connect patients with healthcare, financial, and community resources.
• Help patients understand and apply for benefits such as Medicare, Medicaid, SNAP, and prescription assistance.
• Schedule appointments, referrals, and follow-up care.
• Educate patients on managing their health and chronic conditions.
• Communicate with patients by phone, in person, text, and video.
• Work closely with doctors, nurses, medical assistants, and other healthcare team members.
• Document patient interactions and updates in the EMR.
• Help patients overcome barriers to receiving the care and resources they need.

Required Qualifications:

• Bilingual in English and Spanish.
• High School Diploma or GED.
• 4-5+ years of healthcare or relevant experience.
• Experience in case management, patient navigation, care coordination, or a similar role.
• Experience connecting patients with community resources and government programs.
• Experience assisting with benefit applications such as SNAP, LIS, PAP, or prescription assistance.
• EMR experience.
• Strong communication, organization, and time-management skills.
• Compassionate and patient-focused approach.
• Ability to work effectively with people from diverse backgrounds.

Preferred Qualifications:

• Bachelor's Degree.
• Community Health Worker (CHW) Certification.
• Senior or geriatric healthcare experience.
• CHW certification is required. Candidates without certification must be willing to obtain certification within 6 months.
