Job Summary: Are you looking to move into a more clinical, patient-focused pharmacy role? A rapidly growing specialty pharmacy is seeking a Home Infusion Pharmacist to join its expanding team in West Chester, OH. This is an excellent opportunity for pharmacists from retail, hospital, specialty, infusion, or closed-door pharmacy backgrounds who are interested in working in a collaborative environment focused on improving patient outcomes through home infusion therapy.

Location: West Chester, OH

Pay: $58.00-$65.00/hour

Schedule: Full-Time | Monday-Friday | 8:30 AM-5:00 PM | Onsite

What You'll Do:
  • Review and verify physician orders for home infusion therapies, including IV antibiotics, TPN, specialty biologics, and other infusion medications.
  • Compound or oversee the preparation of sterile IV medications in accordance with USP <797> and USP <800> standards.
  • Verify the accuracy of medications, labeling, and supplies before home delivery.
  • Collaborate with physicians, nurses, and case managers to ensure safe, effective patient care.
  • Monitor patient therapy, laboratory results, and make clinical recommendations when appropriate.
  • Educate patients and caregivers on medication administration, storage, and safety.
  • Supervise pharmacy technicians and support daily pharmacy operations.
  • Maintain compliance with all state, federal, and accreditation standards.
Qualifications:
  • Active Ohio Pharmacist license in good standing.
  • PharmD or Bachelor of Science in Pharmacy from an accredited program.
  • Strong clinical knowledge and commitment to medication safety.
  • Sterile compounding or clean room experience is preferred but not required.
  • Hospital, specialty, infusion, retail, or closed-door pharmacy experience welcomed.
