Responsibilities/Accountabilities
Our client has an amazing opportunity for Social Worker that can:
Acts as facility liaison with county social workers, community services boards, Medicaid, SSI, Social Security, legal aide and other services
Performs assessments of residents
Experience/Skills/Education
Required:
Master's degree (M.S.W.) in Social Work from accredited University and a minimum of five years related experience and/or training, or equivalent combination of education and experience.
Out client can offer free supervision to become LCSW, or LPC to the qualify Candidate.
Experience working with the elderly in an adult living or long-term care facility with knowledge of geriatric mental health services.
Knowledge of Medicaid/Medicare eligibility requirements, PDPM, MDS, Care Planning and basic medical terminology is required.
Proficiency in computer skills, including MS Office, and Outlook.
