Doctor of Pharmacy (PharmD) degree, active Pharmacist license, and minimum 1+ years Pharmacist experience required. Applicants who do not meet these qualifications will not be considered.

A home infusion clinical pharmacist plays a vital role in preparing, dispensing, and monitoring complex intravenous (IV) and subcutaneous medications for patients receiving care at home. This position serves as the central hub of a multidisciplinary team, ensuring safe medication use through sterile compounding, dose adjustments, and comprehensive patient education.

Responsibilities:

  • Prepare specialty pharmaceuticals and IV solutions in a sterile, ventilated environment adhering to all state and federal regulations.
  • Assess lab values, perform pharmacokinetic dosing calculations (e.g., vancomycin), and monitor Total Parenteral Nutrition (TPN) therapy.
  • Provide detailed counseling to patients and caregivers on specialty medications, infusion pumps, sterile techniques, and catheter maintenance.
  • Collaborate with prescribing physicians, home health nurses, and pharmacy teams to coordinate patient care and minimize hospital readmissions.
  • Maintain compliance with State Board of Pharmacy regulations, HIPAA, FDA, DEA, Medicare, ACHC, URAC, OSHA, and other relevant regulatory requirements.
  • Manage pharmacy inventory, including refrigerated products, and prepare medications and supplies for safe, timely home delivery.
  • Utilize CareTend system for clinical and operational documentation.

Qualifications & Desired Experience:

  • AZ and TX pharmacist licenses current and in good standing.
  • 1 to 3 years of experience in infusion specialty pharmacy or hospital pharmacy with primary responsibilities in an IV room.
  • Experience with IVIg therapy is a plus.
  • Familiarity with specialty therapy payer requirements, reimbursement processes, and contract obligations.
  • Strong communication and collaboration skills to work effectively with multidisciplinary teams and patients.

Schedule:

  • Monday – Friday, 8:30 am to 5:00 pm
  • On-call rotation may be required

Location: Glendale, AZ
