A Staff Clinical/Infusion Pharmacist role is available in Houston, TX, offering an engaging opportunity to contribute to specialized patient care by preparing, dispensing, and monitoring complex intravenous (IV) and subcutaneous medications in a home infusion setting. This contract position begins ASAP and spans 8-12 weeks with a Monday through Friday schedule from 8:30 a.m. to 5:00 p.m.; participation in an on-call rotation may also be required.

Key Responsibilities:

  • Prepare specialty pharmaceuticals and IV solutions in a sterile environment, adhering to all state and federal guidelines.
  • Conduct clinical monitoring by assessing lab values, calculating pharmacokinetic dosing such as for vancomycin, and managing Total Parenteral Nutrition (TPN) to optimize patient treatment.
  • Provide detailed patient counseling and education regarding the use of infusion pumps, sterile techniques, and catheter maintenance.
  • Collaborate closely with prescribing physicians, home health nurses, and multidisciplinary teams to coordinate care and minimize hospital readmissions.
  • Ensure compliance with regulatory requirements including State Board of Pharmacy, HIPAA, DEA, Medicare, and OSHA standards.
  • Oversee pharmacy inventory management and prepare medication shipments to patients’ homes safely and efficiently.

Qualifications:

  • Current and valid Texas pharmacist license in good standing.
  • 1-3 years of experience in infusion specialty pharmacy or hospital pharmacy with IV room responsibilities.
  • Experience with IVIg therapy is advantageous.
  • Familiarity with specialty therapy payer requirements, reimbursement mechanisms, and contract obligations.
  • Proficiency with CareTend systems preferred.
