- MD or DO from an accredited institution
- Completion of an ACGME-accredited OB/GYN residency (or expected before starting)
- Board certified or board eligible in OB/GYN
- Eligibility for licensure in Oregon
- Eligibility for DEA and hospital privileges
- Insurability for malpractice coverage
