Your Responsibilities

Deliver Exceptional Patient Care

• Administer direct physical therapy services under the supervision of a licensed Physical Therapist.
• Implement personalized care plans to assist patients in alleviating pain, enhancing mobility, and restoring function.
• Guide patients through therapeutic exercises, gait training, balance activities, and functional mobility.
• Support patients with transfers, strength training, and daily living activities.
• Apply therapeutic modalities such as heat, cold, ultrasound, and electrical stimulation as needed.
• Educate patients and their caregivers on exercises, safety measures, and the use of assistive devices.

Track Progress & Communicate

• Monitor and report on patient progress, treatment responses, and any changes in their condition.
• Ensure accurate and timely documentation in the electronic medical record (EMR).
• Maintain effective communication with the Physical Therapist and interdisciplinary care team.
• Engage in case conferences and care coordination activities.

Collaborate & Contribute

• Work closely with nurses, therapists, and other healthcare professionals to provide coordinated care.
• Participate in team meetings, training sessions, and ongoing professional development.
• Adhere to clinical standards and contribute to quality improvement initiatives.

Your Qualifications

• Graduation from an accredited Physical Therapy Assistant (PTA) program.
• Current PTA license or certification as mandated by state regulations.
• Preferred experience of 1+ years (home health experience is a plus).
• Current CPR certification.
• A valid driver’s license, dependable transportation, and auto insurance.
