Overview:
\n Our multi-specialty practice is seeking a Family Nurse Practitioner to provide primary care for pediatric and adult patients. Our flexible full-time or part-time opportunity offers a collaborative environment, no call responsibilities, and welcomes new graduates.

Position Offers:

\n
• Schedule: Full-time (4 weekdays plus Saturday) or part-time (3 days per week)
\n
• Competitive salary: $125,000–$135,000 per year (DOE)
\n

• No call responsibilities
\n

Key Responsibilities:

\n
• Conduct new patient consultations, follow-up visits, and medication management
\n
• Perform comprehensive assessments for pediatric, adolescent, adult, and geriatric patients
\n
• Diagnose, treat, and manage acute and chronic conditions
\n
• Develop and implement individualized care plans for chronic diseases, including diabetes, hypertension, and cardiovascular conditions
\n
• Provide preventive care, including well-child visits, adult physicals, immunizations, and health screenings
\n
• Educate patients and families on disease prevention, medication adherence, and self-management
\n
• Coordinate care and referrals with physicians and other healthcare professionals while maintaining accurate EHR documentation
\n
• Apply evidence-based clinical guidelines and protocols to support safe, high-quality patient care
\n

Qualifications:

\n
• Active New York State Nurse Practitioner license
\n
• Master's degree in Nursing with a Family Practice focus
\n
• National Family Nurse Practitioner certification (AANP or ANCC)
\n
• Current DEA registration or eligibility to obtain
\n
• BLS certification
\n
