Physical Therapist (PT)

Location: Skilled Nursing Facility | Grand Junction, CO

Compensation: $40.00 - $55.00 per hour, depending on experience + $15,000 Sign-On Bonus

Key Responsibilities:

• Conduct thorough physical therapy evaluations for residents.
• Create personalized treatment plans tailored to resident needs, goals, and physician directives.
• Evaluate and recommend assistive devices, mobility aids, and safety enhancements.
• Track resident progress and modify treatment plans as necessary.
• Provide education to residents, families, and caregivers regarding treatment strategies and discharge plans.
• Maintain precise and timely documentation in line with regulatory and professional standards.
• Engage in interdisciplinary care planning meetings.
• Ensure adherence to all federal, state, facility, and professional practice regulations.

Qualifications:

• Active Colorado Physical Therapist (PT) license in good standing.
• Preferred experience in skilled nursing facilities, rehabilitation, or long-term care.
• Strong skills in clinical assessment and treatment planning.
• Dedication to delivering outstanding resident-centered care.
• Experience in skilled nursing facilities (SNF).
• Familiarity with orthopedic rehabilitation, neurological conditions, balance training, fall prevention, and post-surgical recovery.
• Knowledge of Medicare Part A, Part B, and managed care rehabilitation services.
