Your Role

As a Home Health Occupational Therapist, you will be essential in assisting patients to regain their independence, enhance their daily functioning, and safely manage their lives at home. You will work closely with patients, their families, and interdisciplinary teams to provide tailored, impactful care.

Patient Care & Clinical Responsibilities
  • Assess patients’ functional abilities, including mobility, strength, coordination, communication, and daily activity performance.
  • Evaluate home settings to identify safety hazards and suggest modifications or adaptive equipment.
  • Create personalized treatment plans with specific, measurable objectives aimed at enhancing independence and quality of life.
  • Deliver therapeutic interventions to restore functionality, including retraining in daily living tasks.
  • Design and fit splints, and instruct patients on the use of adaptive, prosthetic, and orthotic devices as needed.
  • Assist patients in returning to work through functional and work capacity assessments.
  • Educate patients, families, and caregivers about treatment plans, safety, and ongoing care requirements.
  • Collaborate with the care team to coordinate services and ensure a seamless, patient-focused experience.
  • Engage in discharge planning to facilitate successful transitions and continued progress.

Care Coordination & Communication
  • Accurately document patient care in a timely manner within the electronic medical record (EMR).
  • Communicate care plans with physicians and interdisciplinary team members.
  • Participate in case discussions, team meetings, and ongoing clinical collaboration.
  • Maintain strong, professional relationships with patients, families, and team members.

Leadership & Oversight
  • Supervise Certified Occupational Therapy Assistants (COTAs) in compliance with state regulations and organizational policies.
  • When therapy is the only skilled service, oversee home health aide care as necessary.

Qualifications
  • Graduation from an accredited Occupational Therapy program.
  • Current registration with the National Board for Certification in Occupational Therapy (NBCOT).
  • Active state licensure as an Occupational Therapist.
  • Preferred minimum of one (1) year of clinical experience (home health experience is a plus).
  • Current CPR certification.
  • Valid driver’s license, reliable transportation, and auto insurance.
