Position OverviewThe physical therapist is responsible for assessing, planning, organizing, and engaging in rehabilitation programs aimed at improving mobility, alleviating pain, enhancing strength, and preventing or reducing deformities in patients affected by illness or injury. Services are delivered throughout the entire care continuum, focusing on quality, cost-effectiveness, and customer satisfaction. Adherence to all relevant regulatory standards is essential in this role.
Key Responsibilities-
Treatment Planning and Care Delivery:
- Implements the designated physical therapy treatment plan.
- Selects suitable treatment activities to help patients achieve their goals.
- Adjusts goals and care plans based on input from patients and their families.
- Provides continuous education and training for patients and their families.
- Offers discharge instructions and referrals to community resources as needed.
- Serves as a resource for staff, families, and physicians regarding rehabilitation services and equipment.

Patient Assessment:
- Collaborates with physicians to establish and evaluate treatment programs, care plans, goals, and discharge strategies tailored to the patient's physical therapy needs.
- Develops a care plan in partnership with the patient and/or their family members.
- Creates a realistic discharge plan that incorporates the goals of the patient and/or family members.

Documentation:
- Records evaluations and care plans in accordance with departmental guidelines.
- Documents each treatment session following departmental protocols.
- Completes and submits required documentation, including family education and discharge information, in line with facility and program standards.
- Upholds ethical business practices by striving to meet the highest standards of integrity and honesty.
- Embraces diversity and demonstrates the ability to collaborate effectively.
- Maintains confidentiality regarding all patient information.
- Follows attendance and punctuality policies as per organizational guidelines.
- Adheres to the departmental dress code, including proper display of ID badges.
- Communicates effectively, both verbally and in writing, with all team members.
- Participates in staff meetings, required training sessions, process improvement initiatives, and other meetings as needed.
- Organizes workload to fulfill responsibilities in a timely manner.
- Utilizes materials and equipment safely and cost-effectively.
- Observes infection prevention protocols.

- Completes all mandatory requirements set by the hospital and/or organization.
Performs other responsibilities as assigned and complies with all organizational policies, work standards, and codes of conduct.
Education RequirementsDoctorate, Master's, or Bachelor's degree in Physical Therapy.
Licenses and Certifications- Physical Therapist (PT) license.
- Basic Life Support (BLS) certification or BLS Instructor certification.
- Strong communication skills, a focus on customer service, teamwork orientation, and basic computer proficiency.
