\nRole: Open to Family Nurse Practitioners and Physician Assistants
\nLocation: Neighborhood Health covers various locations across CT. This position requires travel to our home base hubs out of Hartford and New Britain then travel in company fleet to clinic sites and Street Medicine.
\nSetting: 100% Outpatient Primary Care/Urgent Care Family Medicine
\nSchedule: Full-time, weekdays only.

\nNurse Practitioners and Physician Assistants Role Scope:
\nUnder established guidelines and in collaboration and/or supervision with the practice physician(s), Nurse Practitioners and Physician Assistants perform routine physical examinations, and formulate medical, educational, and psychosocial plans to treat patients.
\nIn consultation with the physician(s) and care team members, Nurse Practitioners and Physician Assistants develop plans for further evaluation of health needs, e.g., referral to other health team members or specialists.
\nNurse Practitioners or Physician Assistants provide appropriate treatment for problems within the scope of the providers practice as designated by protocols and identifies problems outside of their scope of practice and refers appropriately to physicians.

Requirements:
\nLicensure: Unrestricted State of Connecticut Medical License (or eligible)
\nTraining: Family Nurse Practitioner (FNP) or Physician Assistant (PA)
\nExperience: 2+ years as a Family Nurse Practitioner or Physician Assistant, preferably in a primary care or urgent care setting.
\nValid drivers License within 3 Months (90 days) of hire date or job transfer date
\nAbility to push/pull/lift 50 lbs
