Qualifications:

• MD or DO from an accredited medical institution
• Completion of an accredited Family Medicine Residency Program
• Eligible for or certified by the American Board of Family Medicine
• Ability to secure an unrestricted Pennsylvania Medical License and necessary credentials
• DEA certification
