PATHS is seeking a Family Nurse Practitioner or Physician Assistant to provide high-quality, patient-centered primary care in our Danville clinic. You'll join an experienced, collaborative care team practicing evidence-based medicine within a Patient-Centered Medical Home (PCMH), caring for patients across the lifespan while making a meaningful impact in the community.

What You'll Do
\nAs a member of the PATHS care team, you'll provide comprehensive primary care while building long-term relationships with patients and families.
\nResponsibilities include:
\n- \n
- Perform comprehensive patient evaluations and physical examinations \n
- Diagnose and manage acute and chronic medical conditions \n
- Develop individualized treatment plans \n
- Order and interpret diagnostic testing \n
- Prescribe medications and manage refills \n
- Coordinate specialty referrals and follow-up care \n
- Educate patients on disease prevention and healthy lifestyles \n
- Review laboratory and imaging results with patients \n
- Maintain timely documentation within eClinicalWorks \n
- Participate in quality improvement initiatives and provider meetings \n
Qualifications
\nRequired
\n- \n
- Licensed Family Nurse Practitioner (FNP) or Physician Assistant (PA) in Virginia \n
- Current DEA registration \n
- Eligible to bill Medicare, Medicaid, and commercial insurance \n
Preferred
\n- \n
- Board Certification \n
- Experience practicing in an FQHC \n
- Experience with eClinicalWorks \n
