Job Description
Doctor of Pharmacy (PharmD) degree, active Pharmacist license, and minimum 1+ years Pharmacist experience required. Applicants who do not meet these qualifications will not be considered.
A clinical pharmacist role focusing on home infusion therapy is available for an immediate start on an 8-12 week contract. This opportunity involves working Monday through Friday from 8:30 a.m. to 5:00 p.m., with a potential on-call rotation. The position requires expertise in preparing, dispensing, and monitoring complex intravenous (IV) and subcutaneous medications, serving as a critical link within a multidisciplinary healthcare team.

Schedule:
- Monday to Friday, 8:30 a.m. – 5:00 p.m.
- Possible on-call rotation
Key Responsibilities:
- Prepare specialty pharmaceuticals and intravenous solutions in sterile, ventilated environments adhering to federal and state regulations.
- Assess laboratory values and calculate pharmacokinetic dosing; monitor and adjust Total Parenteral Nutrition (TPN) therapies.
- Provide detailed patient and caregiver education on medication administration, sterile techniques, and infusion pump use.
- Collaborate with physicians, home health nurses, and various internal teams to ensure effective therapy management and prevent readmissions.
- Maintain strict compliance with regulatory standards including HIPAA, FDA, DEA, Medicare, and other applicable agencies.
- Manage pharmacy inventory including refrigerated medications, coordinating safe packaging and delivery of patient supplies.
Qualifications and Desired Experience:
- Current Arizona pharmacist license in good standing.
- 1-3 years of infusion specialty pharmacy experience or hospital pharmacy experience primarily working in an IV room.
- IVIg experience is advantageous.
- Familiarity with specialty therapy payer requirements, reimbursement processes, and contractual obligations.
- Experience with the CareTend system is preferred.
