As a Home Health Occupational Therapist, you will be instrumental in assisting patients to regain their independence, enhance daily functioning, and navigate their home environments safely.
Key Responsibilities:- Evaluate home settings to identify potential safety hazards and suggest necessary modifications or adaptive equipment.
- Create personalized treatment plans with specific, measurable objectives aimed at enhancing independence and overall quality of life.
- Construct splints and instruct patients on the use of adaptive, prosthetic, and orthotic devices as needed.
- Accurately document patient care in a timely manner within the electronic medical record (EMR) system.
- Collaborate with physicians and interdisciplinary team members to communicate care plans effectively.
- Oversee Certified Occupational Therapy Assistants (COTAs) in compliance with state regulations and organizational policies.
- When therapy is the only skilled service provided, supervise home health aide care as necessary.
- Graduation from an accredited Occupational Therapy program.
- Current registration with the National Board for Certification in Occupational Therapy (NBCOT).
- Active state licensure as an Occupational Therapist.
- Preferred minimum of one (1) year of clinical experience (experience in home health is a plus).
- Possession of a valid driver’s license, reliable transportation, and auto insurance.
