Position Overview

At our organization, team members transform lives and instill hope through genuine care. The Physical Therapy Assistant delivers skilled, compassionate physical therapy services under the supervision of a licensed Physical Therapist. Key Responsibilities

Patient Care

  • Administer physical therapy interventions as directed by a licensed Physical Therapist, following the physician’s care plan and state Physical Therapy Practice Act.
  • Provide treatment to alleviate pain, maintain function, and promote maximum safe performance.
  • Guide patients through active and passive therapeutic exercises, muscle re-education, gait training, functional mobility activities, transfer training, and prosthetic training.
  • Employ physical agents such as heat, cold, water, light, electricity, ultrasound, and massage in accordance with state practice guidelines.
  • Monitor and report patient responses, progress, or changes in condition to the Physical Therapist and relevant agency personnel.
  • Educate patients on the safe use and maintenance of mobility and supportive devices, including wheelchairs, braces, crutches, canes, and prosthetic or orthotic devices.

Communication & Documentation

  • Record clinical notes and updates promptly and accurately; ensure all documentation is included in the patient’s clinical record as required.
  • Engage in interdisciplinary care discussions and collaborate with the hospice care team to facilitate coordinated service delivery.

Additional Duties

  • Attend home care team meetings, in-service training, and interdisciplinary group (IDG) meetings.
  • Participate in quality assessment and performance improvement initiatives.
  • Perform other duties as assigned to ensure high-quality patient care.

Qualifications

  • Graduate of an accredited Physical Therapy Assistant program with an associate degree in science or applied science (approved by APTA).
  • Meets hospice personnel qualifications as per §418.114(b)(8).
  • Current PTA certification or licensure as mandated by state law.
  • Excellent verbal, written, and organizational skills.
  • Current CPR certification.
  • Valid driver’s license with a reliable, insured vehicle that meets organizational standards.
