Location: Mantey Heights Rehabilitation and Care Center, Grand Junction, CO
Compensation: $40.00 - $55.00 per hour, depending on experience + $10,000 Sign-On Bonus

- Conduct thorough occupational therapy evaluations for residents.
- Create and execute personalized treatment plans tailored to individual resident needs and objectives.
- Deliver expert occupational therapy services to residents recovering from illness, injury, surgery, or functional decline.
- Evaluate and recommend adaptive equipment, assistive devices, and necessary environmental adjustments.
- Provide education to residents, families, and caregivers on therapeutic methods and safety protocols.
- Maintain precise and timely documentation in line with facility, state, and federal regulations.
- Engage in interdisciplinary care plan meetings.
- Track resident progress and adjust treatment plans as needed.
- Ensure adherence to all regulatory and professional standards.
- Current Colorado Occupational Therapist license in good standing.
- Preferred experience in skilled nursing facilities, rehabilitation, or long-term care.
- Strong skills in clinical assessment and treatment planning.
- Dedication to providing outstanding resident-centered care.
