Your Role in Patient CareAs a Home Health Physical Therapist, you will be an essential member of the care team, assisting patients in their recovery journey, enhancing mobility, and helping them maintain independence in the environment they cherish most: their home.
Key Responsibilities- Deliver patient-centered care by evaluating and assessing patients’ functional status, mobility, strength, and rehabilitation potential.
- Collaborate with physicians and the care team to develop and implement personalized care plans.
- Provide hands-on therapy aimed at improving mobility, strength, balance, and independence.
- Facilitate safe transitions of care and contribute to discharge planning.
- Work closely with nurses, therapists, and physicians to ensure coordinated patient care.
- Accurately document patient care and progress in a timely manner within the electronic medical record (EMR).
- Supervise Physical Therapist Assistants (PTAs) in compliance with state regulations.
- Graduation from an accredited Physical Therapy program (DPT or Master’s).
- Current Physical Therapy license in the state of practice.
- A minimum of 1–2 years of experience, preferably in home health.
- Possession of a valid driver’s license, reliable transportation, and auto insurance.
