As a Home Health Physical Therapist, you will plan, organize, and deliver therapy services aimed at maintaining comfort, function, and quality of life, rather than focusing solely on rehabilitation. You will guide patients and caregivers through supportive techniques, safe mobility practices, and meaningful education to promote dignity and ease during end-of-life care. Key ResponsibilitiesPatient Care- Deliver physical therapy services in accordance with the physician’s care plan and state regulations.
- Assess and evaluate therapeutic, rehabilitative, and functional status; contribute to the comprehensive care plan.
- Evaluate the home environment to enhance patient safety and comfort.
- Assess muscle strength, mobility, gait, range of motion, and transfer abilities, focusing on comfort rather than rehabilitation.
- Direct physical therapy treatments and educate patients and families on the use and maintenance of therapeutic devices.
- Identify and prioritize therapy needs, reporting patient responses or changes in condition to the physician.
- Initiate therapy programs and instruct family members and interdisciplinary team members on therapy techniques and care objectives.
- Train patients in the use of prosthetic devices when applicable.
- Recognize the need for additional home health services and make referrals as necessary.
- Provide consultations to patients, families, and caregivers.
- Prepare and submit clinical notes, progress summaries, and discharge summaries in accordance with organizational policies.
- Participate in patient discharge planning to ensure seamless transitions in care.
- Communicate effectively with physicians, caregivers, and interdisciplinary team members.
- Engage in interdisciplinary group meetings to support coordinated hospice care.
- Provide in-service education for nursing and organizational staff as needed.
- Participate in peer consultations and quality assurance activities.
- Supervise Physical Therapy Assistants in accordance with policy and state regulations.
- When therapy is the sole skilled service, instruct, supervise, and evaluate home health aide care biweekly.
- Bachelor’s or master’s degree in physical therapy from an accredited institution.
- Licensed to practice as a physical therapist in the relevant state.
- Preferred two years of relevant experience; experience in home health or community settings is beneficial.
- Strong verbal, written, and organizational abilities.
- Current CPR certification.
- Valid driver’s license with an insured vehicle that meets organizational standards.
