Position Overview

The physical therapist is responsible for assessing, planning, organizing, and engaging in rehabilitation programs aimed at improving mobility, alleviating pain, enhancing strength, and preventing or reducing deformities in patients affected by illness or injury. Services are delivered throughout the entire care continuum, focusing on quality, cost-effectiveness, and customer satisfaction. Adherence to all relevant regulatory standards is essential in this role.

Key Responsibilities

• Treatment Planning and Care Delivery:
  • Implements the physical therapy treatment strategy.
  • Selects suitable treatment activities to help patients achieve their goals.
  • Adjusts goals and care plans based on input from patients and their families.
  • Provides continuous education and training for patients and their families.
  • Offers discharge instructions and referrals to community resources as needed.
  • Serves as a resource for staff, families, and physicians regarding rehabilitation services and equipment.
• Patient Assessment:
  • Collaborates with physicians to establish and evaluate treatment programs, care plans, goals, and discharge strategies based on patients' physical therapy needs.
  • Creates a care plan in partnership with the patient and/or their family members.
  • Develops a realistic discharge plan that incorporates the goals of the patient and/or family members.
• Documentation:
  • Records evaluations and care plans in accordance with departmental guidelines.
  • Documents each treatment session following departmental protocols.
  • Completes and submits required documentation, including family education and discharge information, in line with facility and program standards.

Education and Qualifications

• Required Minimum Education: Doctorate, Master's, or Bachelor's degree in Physical Therapy.
• Required Licenses and Certifications:
  • PT - Physical Therapist
  • BLS - Basic Life Support or BLS-I - Basic Life Support Instructor
• Required Experience: Relevant experience in the field.
• Required Skills: Strong communication abilities, customer service orientation, teamwork skills, and basic computer proficiency.
