Job Opportunity: Nurse Practitioner for Senior Primary Care

Our organization is in search of a dedicated Nurse Practitioner to deliver comprehensive primary healthcare services to adults aged 65 and older. In this position, you will focus on providing preventive, chronic, and acute care, with a strong emphasis on promoting wellness, patient education, and fostering long-term relationships with patients.

You will work collaboratively with a multidisciplinary team to ensure the delivery of value-based, patient-centered care, aimed at enhancing the independence and quality of life for seniors.
