Job summaryWe are advertising three Consultant Geriatrician posts:1 WTE Consultant Geriatrician, Senior Health post working within the Acute Senior Health Unit (ASHU) and OPAL teams, providing specialist geriatric input to support the assessment, management, and ongoing care of older people with complex needs in the acute setting.2 WTE Virtual Ward Consultant Geriatrician post supporting the delivery of an integrated frailty pathway across acute and community services, with a core focus on the development and delivery of the Hospital at Home model, enabling high-quality care closer to patients' homes.We welcome candidates to submit a single application for these posts and indicate whether they would like to be considered for one or both roles, including their preferred post(s).Main duties of the job1 WTE Consultant Geriatrician, Senior Health postThey will hold clinical responsibility for half of the patients on this 28 bedded ward. With the multidisciplinary team, the appointee will provide Geriatric Medicine and comprehensive geriatric assessment to older adults with frailty admitted to the ward aiming for rapid, safe discharge home through integrated working with the local community trust.2 WTE Virtual Ward Consultant Geriatrician postThese postholders will lead reactive frailty care across St George's and Wandsworth & Merton CLCH (Community Trust) by providing clinical leadership and senior decision-making across a joint caseload. They will be supported by an MDT from a range of professional backgrounds including ACPs, GP, nurses, therapists and admin.
